End-of-Life Care

Hospice Dufferin Staff and Volunteers reach out to our clients wherever they are located...... family home, hospital, long-term care or retirement home. We promote people enjoying life until the very end.  We believe that a well supported family is able to grieve easier knowing that their loved one had their wishes filled.

Our Social Worker is available for:
  • Supportive counselling 
  • Assisting in understanding what is needed for advance care  planning
  •  Connecting clients and family to other services
  • Assistance with finding financial streams
  • Advocacy 
  • Supporting those difficult conversations with family and friends


Volunteers Offer:

  • Individual visits and phone calls providing ongoing emotional and mental support for clients, family members and other caregivers 

  • Simple comfort measures and relief for caregivers are also part of our service

  • Assistance with writing life stories or legacy letters

  • Creating memories

  • Complementary Therapy - Reiki 




We have a variety of group programs to increase social isolation, education, and provide caregiver relief.  They include:


  • Meet and Greet: A social morning program 

  • Crafting for Wellness: Bring your knitting/crocheting for an old fashioned afternoon out

  • Art Therapy: Express and explore your emotions through art 

  • Look Good- Feel Better: For woman. Come out for an afternoon of pampering

  • Therapeutic Yoga: Yoga postures and practice aimed towards the treatment of health conditions. Involves instruction in yogic practices and teachings to prevent reduce or alleviate structural, physiological, emotional and spiritual pain, suffering or limitations.

  • Night Out: For clients and their caregiver 

  • G.R.I.E.F: Safe place for anyone experiencing bereavement or loss to meet others experiencing similar situations. It is a place to make friends, gain support from people who understand, and to have some fun.
